  • Anyone who has a life-limiting illness with a life expectancy of 6 months or less.
  • Certified by their attending physician and hospice physician.
  • The patient or representative chooses comfort care instead of curative or aggressive treatment.

Admitting a patient to hospice is a simple process. Just call the hospice.

  1. Call (424) 263-5446.
  2. A Licensed Nurse or Physician visits the patient.
  3. Information is presented to patient/family.
  4. If elected, the nurse arranges start of services immediately.
